*As a new or an existing patient to MJBeautyRN Membership Program, I am aware that my first treatment will be charged at the original rate. If you are an existing patient, you can enroll immediately if you had a treatment in the past 30 days. This membership recurs monthly unless canceled. A 12-month minimum commitment is required. A $300 early cancellation fee will apply unless proof of moving 50 miles outside the Chandler area. Service must be used in the month earned or it will be forfeited. We reserve the right to cancel or change this membership at any time without notice. We reserve the right to refuse membership to anyone at our discretion. All treatments and benefits from this membership are non-transferable. A treatment is earned every 3 months after your enrollment date. Lastly, I acknowledge that any additional treatments on the day will be charged at standard rate.
